Onchocerciasis
Strongyloidiasis
Scabies (crusted or if topical treatment has failed or is contraindicated)
Rosacea (papulopustular), see Ivermectin (skin)

Cutaneous larva migrans
Lymphatic filariasis, usually with albendazole (seek specialist advice)
